By booking this service, I confirm that my insurance is NOT the red, white, and blue Medicare card, also known as Original Medicare and not Medicaid or other federally- or state-funded insurance/health program. If your insurance IS the red, white, and blue Medicare card, also known as Original Medicare, or Medicaid, or other federally- or state-funded insurance/health program, we will not be able to accommodate your appointment request at this time.
